Healthy Behaviors-Diabetes Clinic Day
The Diabetes Clinic Day Incentive Program is focused on members 18 -75 years old with Diabetes (Type 1 or 2) who attend and participate in a scheduled clinic day and have one or more of the following services provided: HbA1c Screening Result, medical attention for Nephropathy, Retinal or Dilated Eye Exam and/or a compliant Blood Pressure Reading. The purpose of this program is to emphasize the importance of comprehensive diabetes care and make available a bundle of CDC services (HbA1c, eye, foot exams) at a single location.
Members who attend the Clinic Day and complete Comprehensive Diabetes Care (CDC) will receive a $20 gift card upon confirmation of the completed visit. Members are made aware of the Diabetes Clinic Day opportunities through their primary care of behavioral health providers. A “Clinic Day” is defined as a scheduled health care event connecting Magellan members with a PCP for completion of the annual wellness visit.
Diabetes is defined as a chronic and complex illness requiring continuous medical care. Left unmanaged, diabetes can lead to serious complications including heart disease, stroke, hypertension, blindness, kidney disease, diseases of the nervous system, amputations, and premature death. Offering a one stop shop for diabetic care is a beneficial approach to providing systematic, ongoing patient care.
Participation and Completion Rates:
July 1, 2019 – June 30, 2020, a total of (33) members enrolled and participated in the Adult Diabetes Clinic Days; a total of (33) members (100%) completed their annual wellness visit and earned an incentive.
