Healthy Behaviors-Weight Management
Magellan Complete Care utilizes Independent Living System’s Nutritional Counseling Program that is compliant with the Academy of Nutrition and Dietetics guidelines. Members will receive individualized nutrition plans from ILS Registered Dieticians that include telephonic nutritional counseling, assessments and education. Through participation in the program, members will increase their knowledge, learn how to set nutrition goals, and engage in healthy behaviors. MCC will conduct outreach with the member’s provider regarding their participation in the program and their goals. MCC will request the provider monitor the member’s weight loss and provide education. Along with monitoring the member’s weight loss the provider will educate the member about the health risks of obesity and the benefits of healthy nutrition and weight loss. When members provide MCC with documentation from the provider that they have achieved a 5% weight loss, they will receive a $20 pre-paid Walmart gift card incentive. When members provide MCC with documentation from the provider that they have achieved the goal of 10% weight loss, they will receive a $20 pre-paid Walmart gift card reward.
To identify members who will be referred to the weight management program, providers will complete an assessment of the member using the National Heart, Lung, and Blood Institute Body Mass Index Table posted on the MCC website. MCC providers, ICCMs and Health Guides will document the verbal agreement from the member for participation in the program. MCC members can also go to the Magellan Complete Care website where they can learn about the program and access enrollment information.
Excess weight is often accompanied by high blood pressure, high blood cholesterol, type 2 diabetes, coronary heart disease, and other health problems. Weight reduction is recommended in major guidelines as an initial intervention in the treatment of hypertensive patients. According to the Centers for Disease Control and Prevention website, no matter what your weight loss goal is, even a modest weight loss, such as 5 to 10% of your total body weight, is likely to produce health benefits, such as improvements in blood pressure, blood cholesterol, and blood sugars.
Program Participation and Completion Rates:
From July 1, 2019 – June 30, 2020, a total of (34) members enrolled and participated in the Weight Management program. During this period, 1% of members (2 members) participating in the program completed program milestones and earned an incentive.
